    Liability Release Statement

    This agreement releases Skyhigh Entertainment, LLC – DBA Epic Battle Sportz from all liability relating to injuries that may occur during the game of laser tag or any activity associated with the activity. By signing this agreement, I agree to hold Skyhigh Entertainment, LLC – DBA Epic Battle Sportz entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence.

    I also acknowledge the risks involved in playing the game of laser tag or any activity associated with the activity. These include but are not limited to cuts, bruises, sprains, strains, breaks, head trauma or any other injury associated with the participation of a physical sport. I swear that I am participating voluntarily, and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity.

    By signing below I forfeit all right to bring a suit against Skyhigh Entertainment, LLC – DBA Epic Battle Sportz for any reason. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed.

    I hereby agree to sign this document digitally and by doing so release liability on my own behalf or on behalf of minors whom I am their parent or legal guardian.
